Caregivers are overwhelmed and burned out.

  • Approximately 65 million Americans, accounting for 29% of the U.S. population, provide care for a chronically ill, disabled, or aged family member or friend annually.

  • The economic value of unpaid caregiving services is estimated at $610 billion annually.

  • Elderly caregivers experiencing stress related to caregiving have a 63% higher mortality rate compared to non-caregivers.

  • In 2010, there were 7.2 potential family caregivers (aged 45–64) for every person aged 80 or older. By 2030, this ratio is projected to decline to 4:1, and by 2050, it will further decrease to 3:1.

Provide them with the tools they need.

  • Automate caregiver enrollment, daily notes, and incident reporting—so your SFC documentation is timely, compliant, and audit-ready.

    • Real-time Communication and Coaching: Connect with professional care coaches experienced in supporting caregivers across various conditions through secure video calls and instant messaging (secure and immediate communication).

    • Collaboration and Coordination: Facilitate communication and coordination among family members, healthcare providers, and other members of the care team.

    • Symptom Tracking and Management: Track a range of symptoms relevant to different chronic illnesses to identify patterns and manage care more effectively. This includes validated assessment tools and the ability to monitor changes over time.

    • Medication Management: Set up medication reminders and maintain records of medications, dosages, and schedules, adaptable for complex medication regimens across different conditions.

    • Personalized Care Plans: Create and manage customized care plans tailored to the specific needs of the individual receiving care, regardless of their condition.

  • CareNotes utilizes a modular software platform to support various condition-specific modules over time.

In partnership with Indiana University and the Regenstrief Institute, today CareNotes provides support for dementia, and we are actively expanding to include conditions like cancer and liver disease.

Over time, CareNotes can provide a comprehensive platform to support caregivers who are caring for loved ones who have two or more chronic health conditions at the same time.

  • Realtime access to education, resources, and coaching that prioritizes the caregiver’s own health and well-being.

Access to clinically supported, tailored, and bite-sized information based on the specific needs of the caregiver and the condition(s) of the care recipient.

Ability to provide support for CE credits and requirements.
